Measuring Marketing When Patients Never Click

When Google answers on the results page, patients can choose you without ever touching your website. Here's the new scoreboard: GBP calls, direction taps, booking clicks, and branded search volume, with the math to prove it.

Last spring, a chiropractor called me ready to fire his marketing company. Website traffic was down 18% year over year, and the monthly report looked like a crime scene. Then I asked his front desk to pull the phone log: new-patient calls were up 22% over the same stretch. Nobody had noticed, because nobody was measuring the thing that actually mattered.

This is the uncomfortable arithmetic of zero-click search: when Google answers on the results page, a patient can choose you without ever touching your website. If visits are your scoreboard, your best month can look like your worst.

The Old Scoreboard vs. the New One

The Four Numbers That Matter Now

Three of these live in one place: the performance tab of your Google Business Profile, which most chiropractors have never opened. The fourth lives in Google Search Console, which is free and takes ten minutes to connect. None of them require an agency, a subscription, or a single line of code.

  1. Calls from your profile. Your Google Business Profile performance dashboard counts every tap on the Call button. This is the closest thing you have to a recommendation counter—each one is a patient who was convinced on Google's page, not yours.
  2. Direction taps. Nearly pure intent. Nobody requests directions to a chiropractor out of idle curiosity; a direction tap is a body headed toward your table.
  3. Booking clicks. Taps on your booking link straight from the results page. If you don't have online booking wired into your profile, you're invisible to the growing share of patients who won't call anyone.
  4. Branded search volume. In Search Console, filter queries containing your practice or doctor's name. When AI overviews and word of mouth recommend you, people search your name to verify. Rising branded volume is the earliest signal your zero-click presence is working—in my experience it moves weeks before revenue does.

Here's the Math

A real month, anonymized:

Website sessions: 412 (down 9% -- "bad month"?)

GBP calls: 63
Direction taps: 41
Booking clicks: 28
                         ---
Patient actions: 132

At a 40% call-to-appointment rate, that's ~53 booked
opportunities -- from a month the traffic graph
called a decline.

Grade that month on sessions and you'd cut the budget. Grade it on patient actions and you'd double down. Same month, same marketing—different scoreboard.

One HIPAA flag before you instrument anything: if you add call tracking to count those calls, recordings of patients describing symptoms are protected health information. Use a vendor that will sign a Business Associate Agreement, or track call counts without recording audio.

The 20-Minute Monthly Dashboard

On the first of each month, pull five numbers: the four above, plus website sessions—kept for context, not judgment. Drop them in a spreadsheet next to the prior three months. Ninety-day trends mean something; single-month swings mostly don't. If a number falls two months running, that's your cue to dig in—starting with whether your hours, services, and structured data still match reality. Twenty minutes, once a month, and you will know more about your marketing than the average practice learns from a 30-page agency report.

The takeaway: patients haven't stopped choosing you. They've stopped announcing it to your analytics. Move your scoreboard to where the choices actually happen—the results page—and the rest of the zero-click pillar starts making sense. The full playbook for earning those calls and taps lives on the Schema & Zero-Click Search hub.

And you can't improve a scoreboard you've never read. Start with the free On-Page SEO Audit to confirm your site is feeding the zero-click machine at all—then watch the four numbers above for 90 days.
